Abstract

BackgroundOver the last 20years the oncological results in the treatment of vulvar cancer have not been improved. Due to a lack of evidence from randomized studies many aspects of treatment are unclear and controversial. From aradio-oncological perspective the aims of treatment are the avoidance of local and locoregional recurrences, improvement of organ and function preservation within the framework of the interdisciplinary treatment concept and avoidance of unnecessary toxicity.MethodAliterature search was carried out and asystematic presentation of the current treatment standards in the radio-oncological treatment of vulvar cancer is given.ResultsTumor-infested resection margins necessitate alocal follow-up resection or, if this is not possible or functionally unfavorable, postoperative (chemo)radiotherapy. The current German guidelines recommend aresection of at least 3mm into healthy tissue. The prevention of inguinal recurrences improves the disease-free survival. The frequently carried out practice of pelvic irradiation is overtreatment in approximately 70%, even for patients with positive inguinal lymph nodes. In this case areevaluation has become established for avoidance of unnecessary toxicity. The current German guidelines recommend asurgical staging of the pelvis and pelvic irradiation only in cases of positive lymph nodes.ConclusionTopics of discussion and future reevaluation are the value of simultaneous chemotherapy, neoadjuvant versus primary chemoradiotherapy, the optimal radiosensitizing substance and the optimal irradiation dosage in primary treatment. An interdisciplinary approach represents an indispensable prerequisite for an optimal treatment.
