Mitral valve regurgitation (MR) is one of the most common valvular heart diseases in adults. Patients may present with symptoms of congestive heart failure but may also be asymptomatic for a long time. There is an important distinction between primary and secondary MR, depending on the underlying mechanism. Primary MR is caused by a structural abnormality of the mitral valve apparatus (for example prolapse). In secondary MR the valve is structurally normal and MR results from a dilatation or otherwise distorted geometry of the left ventricle (mostly due to underlying cardiomyopathy). Transthoracic echocardiography is essential to establish the diagnosis and etiology. Operative mitral valve reconstruction is seen as gold standard in the treatment of primary MR. In secondary MR guideline-directed heart failure treatment is the cornerstone of therapy. Surgical or interventional therapy of secondary MR is established as a complementary symptomatic therapeutic strategy.
