Abstract

Renal insufficiency influences female fertility and is associated with an increased risk for mother and fetus during pregnancy. This risk is directly related to the severity of renal impairment. Pregnant women with mild renal insufficiency and normal blood pressure do not generally face an increased risk, whereas women with severe renal impairment show complications in up to 92% of cases. Thus, women with severe renal insufficiency are advised to wait with pregnancy until they are in a stable condition after kidney transplantation as this dramatically increases the success rate. In patients with kidney transplants planning a pregnancy, immunosuppression should be adjusted to medication that can be safely used during pregnancy. Treatment for arterial hypertensionmight also have to be changed. Patients with glomerulonephritis might exhibit a decrease of renal function during pregnancy, especially in lupus nephritis and renal involvement of systemic sclerosis. Urinary tract infections in pregnancy require differential antibiotic therapy.
