Abstract

NLP im Arzt-Patienten-Gespräch - Theorie, Praxis und Effekte NLP ist ein kommunikativer Werkzeugkasten, der Modelle anbietet, den persönlichen Umgang zu optimieren, die Wahrnehmung zu schärfen, die Flexibilität im Verhalten zu erhöhen und das Handlungsrepertoire zu erweitern. Effektiv, zeitsparend, sofort umsetzbar sind einige Adjektive, die dem NLP als Wirkung zugeordnet werden. Ob die gewünschte Wirkung erzielt wird, habe ich bezogen auf niedergelassene Ärzte und Zahnärzte untersucht. Die zentralen Aspekte meiner empirischen Untersuchung spiegeln sich in den folgenden Fragestellungen wider: 1. Setzt der Arzt im Verlauf der Ausbildung NLP-Modelle im Gespräch mit Patienten ein? Ziel dieser Arbeit ist es zu erforschen, inwiefern Ärzte die kennengelernten Theoriemodelle der NLP-Ausbildung in den Praxisalltag umsetzen. Grundlage dieser Untersuchung war eine NLP-Ausbildung in modularer Form, die aus acht Bausteinen bestand. Mit dem "Verlauf der Ausbildung" ist somit gemeint, ob Veränderungen in der Anwendung der NLP-Modelle von Abschnitt zu Abschnitt zu beobachten waren. Die erste Beobachtungsphase fand vor dem Start der NLP-Ausbildung statt. Neben der Beobachtung als Fremdeinschätzung des Patientengespräches wurden die Ärzte um ihre Selbsteinschätzung gebeten. Ergebnis: Die Anzahl der durchschnittlich eingesetzten Kommunikationsmodelle pro Arzt hat sich im Laufe der Ausbildung fast verdoppelt. Wird das Spektrum der eingesetzten Modelle vor Beginn der Ausbildung (Phase null) mit der Bandbreite am Ende (Phase sieben) und in der Mitte (Phase vier) verglichen, sind die Ergebnisse stark signifikant. Der Trend die NLP-Modelle im Verlauf häufiger einzusetzen, bestätigte sich auch durch die Selbsteinschätzung. Die nicht-komplexen Modelle kamen wesentlich mehr zum Einsatz, da sie das Handlungsrepertoire des Arztes erweitern und in jede Konsultation einfließen können. Gemäß der hier ermittelten Ergebnisse werden die weniger komplexen Modelle als nützliche Hilfsmittel für die zielgerichtete Kommunikation im Patientengespräch eingesetzt. 2. Verändert sich die Arzt-Interaktion durch den Einsatz der NLP-Modelle im Verlauf der Ausbildung? Ich habe vor Beginn und zum Abschluss der Ausbildung einen standardisierten intervallskalierten Fragebogen eingesetzt. Der Fragebogen hat am ersten Messzeitpunkt die erwartete Wirkung der Teilnehmer an die NLP-Practitioner-Ausbildung und am zweiten Messzeitpunkt die erzielte Wirkung erfasst. Ergebnis: Durch die NLP-Ausbildung ist der Arzt in der Lage einen empathischeren Kontakt zu seinen Patienten herzustellen. Das Eingehen auf die Patienten nimmt im Verlauf der Ausbildung zu, was sowohl durch die Beobachtung als auch mittels Fragebogen nachgewiesen werden konnte. Die Teilnehmer gaben an, durch die Ausbildung folgende Wirkungen auf sich selber festgestellt zu haben: erhöhte Zufriedenheit, mehr Energie im Alltag, empathischere Kontaktaufnahme, breiteres Kommunikationsspektrum und informativeres Anamnesegespräch. Die Befragung der Ärzte bezogen auf die Wirkung der NLP-Ausbildung auf die Patienten ergab allerdings, dass sich die Erwartungen nicht erfüllten. Durch die NLP-Ausbildung wurden auch keine Veränderungen auf den Praxisbetrieb von den Teilnehmern wahrgenommen. 3. Wenden Ärzte mit einer psychotherapeutischen Ausbildung ein breiteres Spektrum von NLP-Modellen an? NLP ist durch die Beobachtungen von drei Therapeuten (Satir, Pearls und Erickson) entstanden und hat in diesem Bereich spektakuläre Erfolge hervorgebracht. Es ist daher zu vermuten, dass gerade Ärzte, die eine psychotherapeutische Ausbildung abgeschlossen haben, ein breiteres Spektrum an NLP-Modellen anwenden. Ergebnis: Durch die Beobachtung konnte festgestellt werden, dass die Ärzte mit psychotherapeutischer Zusatzausbildung ein signifikant breiteres Spektrum an Kommunikationsmodellen einsetzten. Die Befragung ergab, dass die Differenz zwischen den mittleren Rängen bei den Meßzeitpunkten drei und sechs nicht mehr signifikant ist. Ergo fällt die Zusatzausbildung im Verlauf der Ausbildung bei den Fragebögen nicht mehr ins Gewicht. Schlußfolgerung: Durch die Untersuchung konnte für eine ausgewählte Anzahl von niedergelassenen Ärzten gezeigt werden, welche NLP-Modelle im Arzt-Patienten-Gespräch eingesetzt werden. Insbesondere die Modelle der ersten vier Ausbildungsbausteine haben sich als sofort in jedem Gespräch anwendbar herausgestellt. NLP als ein umfassendes Kommunikationsmodell bietet dem Arzt mehr Wahlmöglichkeiten für das Patientengespräch. Die Arzt-Patienten-Interaktion wurde durch die Ausbildung hin zu einer empathischeren Kontaktaufnahme und einem verbesserten Eingehen auf die Patienten beeinflusst. Die NLP zugeschriebene Eigenschaft zeitsparend und effektiv zu sein, konnte nicht nachgewiesen werden. Durch die Ausbildung werden die Handlungsspielräume von Ärzten - auch mit psychotherapeutischer Zusatzausbildung - bereichert und weitere nützlichere Impulse gegeben.

NLP in medical consultation - Theory, Practical Experience and Implications NLP is a tool set for human communication; it provides models to optimize human interaction, to sharpen perception, to enhance the flexibility of behaviour and to widen the repertoire of actions. NLP is said to be effective, time saving, immediately usable. This empirical study investigates if the desired effects of NLP are achieved in the practices of established medical doctors and dentists. The central aspects are reflected in the following questions: 1. During their NLP training, do physicians use NLP models in their communication with patients? It is the aim of this study to investigate to what extent physicians apply the newly trained NLP models in their daily practice. The basis of this research was a modular NLP practitioner training course consisting of eight modules. The question was if the use of NLP models changed from one training module to the following. The first observation took place before the beginning of the NLP training in order to study the previous communication behaviour of the physicians. Besides the evaluation of the doctor-patient interaction by an external observer the physicians were asked for an evaluation by themselves. Result: The average number of communication models used per physician almost doubled during the NLP training. A comparison of the spectrum of models used before the beginning of the training (phase 0) with the spectrum used at the end (phase 7) and in the middle (phase 4) shows that the results are strongly significant. The trend of using NLP models more frequently during the course of the training was confirmed by the doctors' self evaluation. The less complex models were applied considerably more frequently since they extend the doctor's repertoire of actions and can easily be applied in any consultation. According to the results of this study, the less complex models are used as useful tools for result-oriented communication in doctor-patient interaction. 2. Does doctor-patient interaction change through the use of NLP models during the course of the NLP training? Before the beginning and after the end of the NLP training a standardized questionnaire scaled by fixed intervals was used. The questionnaire captured the participants' expectations of the NLP practitioner training at the beginning (measuring point 1) and the resulting effects in the end (measuring point 2). Result: NLP training enables physicians to establish a more empathic contact to their patients. Their ability to interact successfully with their patients improves during the NLP training, which was verified by observation as well as by questionnaire. The participants stated that they observed the following effects on themselves by the training: improved satisfaction, more energy in daily life, more empathic contact to others, extended spectrum of communication and more detailed investigation of the patient's medical history. When asked about the effect of the NLP training on the patients, the physicians stated that their expectations had not been met. Also, the doctors did not realize any changes in the daily operation of their practices which could have resulted from their NLP training. 3. Do physicians with training in psychotherapy use a broader spectrum of NLP models? NLP was created on the basis of the observations of three psychotherapists (Satir, Pearls and Erickson) and has shown spectacular successes in the area of psychotherapy. Therefore it is plausible to assume that especially doctors with training in this area will apply a broader spectrum of NLP models. Result: The observations showed that doctors with an additional training in psychotherapy applied a significantly broader spectrum of communication models. The questioning showed that the difference between the middle ranks of the measuring points 3 and 6 was not any more significant. Accordingly, the additional training in psychotherapy loses importance during the NLP training. Conclusion For a selected number of established physicians this study shows what NLP models are employed in doctor-patient communication. Especially the models taught in the first four NLP training modules proved to be immediately applicable in every consultation. NLP as a universal communication model offers more options to doctors for their contacts with the patients. Due to the NLP training the interaction between doctor and patient developed towards a more empathic approach and an improved response to the patient. The qualities attributed to NLP, to be time saving and effective, could not be verified. Nevertheless, the NLP training enriched the doctors' scope - even of those physicians with a prior training in psychotherapy - and gave additional useful impulses.English
