Abstract

Das Neuroblastom ist weltweit die zweithaeufigste maligne Krebserkrankung bei Kindern. Der Einsatz immuntherapeutischer Behandlungsmethoden stellt eine große Herausforderung in der Klinik dar. Im Hinblick auf die Etablierung antikoerperbasierender Strategien ist der Einsatz bispezifischer CD3 x TAA-Antikoerper ein vielversprechender Ansatz. Ihr Potential, eine T-Zellrezeptor-unabhaengige Aktivierung von T-Zellen zu vermitteln und aktivierte T-Effektorzellen gegen den Tumor zwecks Zelllyse zu richten, kann von großem therapeutischen Nutzen sein. Das neurale Zelladhaesionsmolekül NCAM, das nahezu auf allen Neuroblastomzellen exprimiert wird, ist ein fuer die Therapie relevantes target-Antigen. Da die Etablierung von Tumoren maßgeblich von der VEGF/VEGFR-vermittelten Vaskularisierung beeinflusst wird, kann auch hier der Einsatz bispezifischer Antikoerper von immuntherapeutischer Relevanz sein. Daran orientiert, umfasste die vorliegende Arbeit die Herstellung und In-vitro-Charakterisierung bispezifischer CD3 x NCAM (OKT-3/murin x ERIC-1/murin) und (UCHT-1/humanisiert x D29/humanisiert) sowie CD3 x VEGFR-2 (UCHT-1/humanisiert x A7/murin)-Antikoerper im single chain Fv(bsscFv)- sowie single chain (sc)-diabody-Format. Die bsscFv-Antikoerper wurden aus stabil produzierenden Sp2/0-Myelomazellen aufgereinigt. Der r3V bsscFv (UCHT-1 x A7) zeigte in der FACS-Titration auf CD3-positiven Jurkat- und VEGFR-2-positiven PAE/KDR-Zellen eine deutliche Bindung ab einer Konzentration von 1 µg/ml. In der Analyse der Proliferation humaner PBMCs kam zum Ausdruck, dass der r3V allein und in Gegenwart inaktivierter PAE/KDR-Zellen in der Lage war, die Stimulation CD3-positiver T-Zellen ab einer Konzentration von 1 µg/ml zu aktivieren. Des Weiteren war der r3V in der Lage, T- und VEGFR-2-positive PAE/KDR-Zellen zu vernetzen und aktivierte CD8-positive T-Effektorzellen nach Il-2-Stimulation gegen den Tumor zu richten. Die ermittelte spezifische Zelllyse betrug etwa 30 Prozent bei einem Effektor : Zielzellen-Verhaeltnis (E : T-Ratio) von 30. Bei der PBMC-Stimulation mittels des r3V erfolgte diese Effektorfunktion nicht. Ueberraschenderweise war bei der Zytotoxizitaetsanalyse mit VEGFR-2-negativen PAE/FLT1-Kontrollzellen ein aehnlicher Effekt wie auf PAE/KDR-Zielzellen zu sehen. Daten deuten darauf hin, dass dies auf unspezifische Faktoren zurueckzufuehren war. Der r3N bsscFv (UCHT-1 x D29) zeigte in der FACS-Titration auf CD3-positiven Jurkatzellen eine klare Bindung ab einer Konzentration von 1 µg/ml. Auf NCAM-positiven TE671-Zellen hingegen zeigte der r3N erst eine deutliche Bindung ab einer Konzentration von 80 µg/ml. Die Analyse der Proliferation humaner CD56-depletierter PBMCs verdeutlichte, dass der Antikoerper in einer Konzentration von 1 µg/ml und in Gegenwart inaktivierter IMR5-Zellen in der Lage war, CD3-positive T-Zellen zu stimulieren. Die Stimulation war jedoch im Vergleich zur PBMC-Proliferation in Gegenwart NCAM-negativer LS-Kontrollzellen nicht signifikant erhoeht. Dieser Effekt resultierte mutmaßlich aus der schlechten Bindungskapazitaet der anti-NCAM-Domaene des r3N. In Bezug auf die Bindungseigenschaften der anti-NCAM-Domaene war der r3N im Rahmen dieser Studie weder in der Lage, T- und NCAM-positive IMR5-Zellen zu vernetzen, noch aktivierte Effektorzellen nach Stimulation gegen den Tumor zu richten. Die Antikoerperklone UCHT-1 x A7 und UCHT-1 x D29 wurden zusaetzlich im sc-diabody-Format stabil in BHK21-Zellen exprimiert. Die Expression der diabody-Antikoerper konnte auf mRNA- und Proteinebene deutlich nachgewiesen werden. Die spezifische Bindung der sekretierten Antikoerper wurde erfolgreich im FACS auf CD3-positiven Jurkat-, VEGFR-2-positiven PAE/KDR- sowie NCAM-positiven IMR5- und TE671-Zellen verifiziert. Im Gegensatz dazu war der initial untersuchte OKT-3 x ERIC-1 diabody für funktionale Analysen nicht hinreichend exprimierbar. Die Analyse stabil transfizierter BHK21-Klone ergab, dass der Antikoerper zwar auf mRNA-, aber nicht auf Proteinebene nachweisbar ist. Analoge Studien zum ERIC-1 scFv zeigten den gleichen Befund.

Neuroblastoma is the second most common malignancy among childhood cancers. To date immunotherapy remains a major challenge in paediatric oncology. Efforts are made to establish antibody based approaches to treat cancer. One promising approach is the application of bispecific CD3 x TAA antibodies. Owing to their ability to bind to T and tumour antigens simultaneously they harbour the potential to mediate a T cell receptor independent expansion of T cells and subsequently to redirect activated CD8 effector T cells to the targeted tumour and hence are able to elicit a cytotoxic function. The neural cell adhesion molecule NCAM that is abundantly expressed on nearly all neuroblastoma cells is a tumour associated antigen with therapeutic relevance. As tumour development and maintenance is highly influenced by VEGF/VEGFR mediated neovascularisation the application of bispecific antibodies may also play an important role in antiangiogenic immunotherapies. Accordingly, the presented study focused on the generation and in vitro characterisation of bispecific CD3 x NCAM (OKT-3/murine x ERIC-1/murine) and (UCHT-1/humanised x D29/humanised) as well as CD3 x VEGFR-2 (UCHT-1/humanised x A7/murine) antibodies in the single chain Fv (bsscFv) and the single chain (sc) diabody format. The bsscFv antibodies were stably produced in Sp2/0 myeloma cells and purified from the cell culture supernatants. The r3V bsscFv (UCHT-1 x A7) demonstrated a clear binding on CD3 positive Jurkat and VEGFR-2 positive PAE/KDR cells in a FACS titration analysis at concentrations higher than 1 µg/ml. The evaluation of the antibody's potential to stimulate PBMCs revealed that the r3V was able to activate CD3 positive T cells alone and in the presence of inactivated PAE/KDR target cells at concentrations higher than 1 µg/ml. Furthermore the r3V elicited the ability to crosslink T and VEGFR-2 positive PAE/KDR target cells and to redirect activated CD8 effector T cells after Il-2 stimulation to exert their cytotoxic function. Thereby the calculated specific lysis accounted to about 30 per cent at an E : T ratio of 30. When PBMCs were stimulated with the bsscFv r3V instead of Il-2 cytotoxic effects could not be observed. Surprisingly, the assessment of cytotoxicity with VEGFR-2 negative PAE/FLT1 control cells yielded a similiar cytotoxicity as with PAE/KDR target cells. Data indicate that this was due to unspecific responses. The r3N bsscFv (UCHT-1 x D29) exhibited a clear binding on CD3 positive Jurkat cells at concentrations higher than 1 µg/ml. In contrast, the r3N revealed a low binding capacity on NCAM positive TE671 cells and required a concentration of 80 µg/ml to elicit a clear target cell binding. The assessment of the r3N ability to stimulate human CD56 depleted PBMCs demonstrated that the r3N alone as well as in the presence of inactivated IMR5 target cells was able to activate T cell proliferation at concentrations higher than 1 µg/ml. However, the stimulation was not significantly increased in comparison to the stimulation in the presence of NCAM negative LS control cells. This effect presumably resulted from the low binding capacity of the r3N anti-NCAM domain. In regard to the binding character of the anti-NCAM domain the r3N as applied in this study was neither able to crosslink T and NCAM positive IMR5 cells nor to redirect activated effector T cells post stimulation to the targeted tumour cells. Additionally, the antibody clones UCHT-1 x A7 and UCHT-1 x D29 were stably expressed as sc-diabodies in BHK21 cells. The expression of both diabodies was clearly detected on mRNA and protein level. The specific binding of the secreted antibodies on CD3 positive Jurkat, VEGFR-2 positive PAE/KDR as well as on NCAM positive IMR5 and TE671 cells could be successfully verified using FACS analysis. In contrast, the initially characterised OKT-3 x ERIC-1 diabody proved to be insufficiently expressed for functional analysis. The analysis of stably transfected BHK21 clones revealed antibody expression on mRNA but not on protein level. Studies performed with the ERIC-1 scFv showed the same results.English
