Overlengthening is a frequent problem of radial head arthroplasty. The radius is lengthened relative to the ulna leading to increased pressure on the capitellum. Because even small amounts of overlengthening of 1-2 & x202f;mm can lead to early cartilage attrition, recognition of overlengthening is extremely important. However, clinical symptoms as pain and stiffness are unspecific. Radiological diagnostics remains difficult-especially in cases of minor overlengthening. This article discusses diagnostic workup, therapeutic options, and prevention of overlengthening.
