Introduction: Juvenile spondylarthropathy (SpA) describes a group of diseases leading to morbidity and reduced quality of life in affected children including patients classified as enthesitis related arthritis, a category of juvenile idiopathic arthritis (ERA-JIA). Tumour necrosis factor alpha (TNF alpha) plays an important role in the pathogenesis of both synovitis and enthesitis in these diseases. Thus, in patients with disease refractory to nonsteroidal antiinflammatory drugs (NSAIDs), disease-modifying antirheumatic drugs, and corticosteroids, anti-TNF agents are of utmost importance. Areas covered: This review summarizes published studies of anti-TNF agents in juvenile SpA/ERA-JIA with regard to efficacy toward peripheral arthritis, enthesitis and axial involvement as well as safety aspects. Expert opinion: Both etanercept and adalimumab have been studied in open label and controlled trials in juvenile SpA/ERA-JIA patients. Efficacy data show a beneficial response toward peripheral joint disease, enthesitis as well as axial involvement. These data show long-lasting efficacy with high adherence rates to the treatment. Tolerability to these biologics seem to be good over a year-long treatment, with no significant rise in medically-relevant infections and yet no obvious association with malignancies. For other biologics long-term data are relatively scarce.
