Abstract

Background. The number of arthroplasties of the elbow increased by 2.5-fold between 1993 and 2007. The postoperative outcome following total arthroplasty of the elbow depends not only on the design of the prosthesis, the surgical approach and the surgeon's experience but also on the postoperative rehabilitation. Aim. The aim of the current article is to review and summarize the literature regarding rehabilitation concepts following total arthroplasty of the elbow including the expertise of our own clinical practice. Results. The available literature on this topic describe follow-up treatment concepts without undertaking any scientific investigations but the mentioned rehabilitation concepts are similar. Postoperative rehabilitation differs due to the design of the prosthesis as well as the surgical approach and the resulting integrity of the triceps muscle. Following initial immobilization for reduction of swelling and tissue repair, a rehabilitation program with a limited range of motion increasing over 6 weeks is started. Load bearing should be avoided for 12 weeks. The patient has to be educated on the lifetime restriction of load bearing exceeding 5 kg and repetitive loads exceeding 1 kg. Conclusion. Differences in rehabilitation following total arthroplasty of the elbow result from the design of the prosthesis as well as the surgical approach. Surgical approaches preserving the triceps muscle are preferable due to the possibility of a faster postoperative recovery and the reduced risk of postoperative triceps insufficiency.
