Abstract

Background. Artificial joint replacement is a treatment option for osteoarthritis of a metacarpophalangeal or a proximal interphalangeal finger joint. No general guidelines for postoperative treatment of finger joint prostheses exist. However, individualized postoperative treatment involving consistent, close cooperation with the patient and between the surgeon and therapist is required for a good outcome. Methods. We address several aspects of the postoperative treatment of metacarpophalangeal and proximal interphalangeal finger joint prosthesis implantation, which include edema/swelling, scarring, hand therapy, and splinting, and review indications for postoperative treatment of patients. In addition we describe and discuss current treatment concepts and results of metacarpophalangeal and proximal interphalangeal finger joint prostheses. Results. Even with appropriate treatment following implantation of a finger joint prosthesis, postoperative range of motion improves only slightly. However, pain is significantly reduced and outcome evaluation questionnaire scores improve compared to preoperative values. Conclusion. For a good outcome of a finger joint prosthesis, postoperative attention to edema/swelling, scarring, hand therapy, and splinting is essential. Postoperative treatment of a finger joint prosthesis should follow guidelines but be individually adapted to the approach to the joint, the type of prosthesis, the quality of bone and stability of the soft tissue of the finger, the patient's compliance, and the availability of a qualified therapist.
