Abstract

Background. The titanium elastic nail (TEN) in midshaft-clavicular fractures is an established alternative procedure to plate osteosynthesis. Typical complications are the over-extending medial end, lateral perforation, and telescoping. Methods. Since 1996, 233 patients have been evaluated: 31 % patients developed pain at the medial end of the TEN, but only 1 % of them required prompt shortening of the overextending TEN. In 5 % of the cases, telescoping was detected, whereby lateral perforation was found postoperatively in 2 % of patients. These complications were caused by incorrect classification of the fracture with inadequate stabilization of the two main fragments. In cases of shortening of the clavicle, painful pseudarthrosis or malunion, an operative procedure with locking contact plates was performed. Conclusion. In summary, the TEN is a safe and useful procedure compared to plate osteosynthesis in midshaft clavicular fractures of type A or B according to the OTA classification.
