Abstract

Pseudoachondroplasia (PSACH) and multiple epiphyseal dysplasia (MED) are two constitutional skeletal diseases, which lead to disproportionate dwarfism. MED is the clinically milder disease. Both forms are mostly associated with mutations in the gene for the cartilage oligomeric matrix protein (COMP). These mutations often lead to intracellular retention of the protein in the endoplasmic reticulum (ER). The physiological function of COMP and its role in the pathogenesis of chondrodysplasias is largely unknown. In the present study the effects of a panel of disease-causing mutations (D361Y, D469, D475N, H587R) were examined, initially in bovine and murine cell culture systems. Primary chondrocytes were transfected or transduced with myc-tagged wildtype or mutated COMP constructs and examined in monolayer or three dimensional alginate culture. It was found that the pathogenesis is more diverse than previously thought. PSACH and MED are not exclusively storage diseases, but extracellular mechanisms also play a role in pathogenesis. While all analyzed aspartate mutations showed a disturbed secretion and caused an enlarged ER, the secretion of H587R COMP was unaffected. Despite the retention all aspartate mutant proteins reach eventually the extracellular matrix. Chondrocytes expressing PSACH-associated COMP variants (D469, D475N, H587R) lack well defined distinct collagen fibrils in the extracellular matrix. Instead amorphous aggregates are formed. The MED-associated mutation (D361Y) gave a mild retention and limited matrix alterations. This implies a connection between the cellular phenotype and disease characteristics. Apoptosis was elevated by all mutant proteins irrespective of the secretion behaviour and extracellular matrix phenotype. On ultrastructural level the chondrocytes showed intracellular characteristic channges of apoptosis. For analysis on molecular level the COMP variants were expressed recombinantly. Subtle structural alterations were revealed in dependence of calcium concentration. These changes affect the function of COMP as a chaperone in collagen fibrillogenesis, although the apparent affinity is only moderately altered. While the fibrillogenesis of collagen I and II/XI is delayed due to aspartate mutations, it is accelerated by H587R COMP. However fibrils formed in the presence of H587R COMP display a disrupted structur. This is consistent with the matrix alterations observed in the cell culture model. In cell adhesion assay it was found that extracellular mutant COMP influence cell behaviour. In contrast to the predominant opinion, the pathogenesis of PSACH and MED includes extracellular as well as intracellular mechanisms. Both pathways contribute to an increased cell death and a disintegration of the cartilage matrix which may cause the dwarfism.English
