Abstract
Als es in Deutschland am 16.03.2020 zum ersten Lockdown aufgrund der Covid-19-Pandemie kam, wurden auch die Besuchsregelungen in der Geburtshilfe und in Kinderkliniken eingeschränkt. Ziel dieser Studie war es, mögliche Auswirkungen dieser Zugangsbeschränkungen auf Eltern von Frühgeborenen und kranken Neugeborenen hinsichtlich Stillverhalten, psychischem Wohlbefinden, Belastung und Bindung zum Kind zu untersuchen. Es handelte sich um eine monozentrische, deskriptive und prospektive Studie, die zwischen dem 13.08.2021 und 13.07.2023 durchgeführt wurde. Verglichen wurden zwei Kohorten: Vom 16.03.2020 bis 18.05.2020 durften Eltern die Kinderklinik der Uniklinik Köln nur getrennt voneinander besuchen, ab dem 19.05.2020 wieder gemeinsam. Nach Einwilligung erhielten die Eltern validierte Fragebögen, darunter F-SozU, EBI, IES-R, EPDS und PBQ, sowie selbst entwickelte Fragebögen zu sozioökonomischen Daten und Stillverhalten. Kindliche Daten wurden aus den klinikinternen Datenbanken erhoben. Die Auswertung erfolgte mit SPSS Version 29. Insgesamt nahmen 21 Eltern aus der Phase mit strengen und 15 aus der Phase mit gelockerten Zugangsbeschränkungen teil. Die Studiengruppen waren hinsichtlich der kindlichen und elterlichen Basisdaten sowie der sozioökonomischen Situation vergleichbar. Beide Gruppen verfügten über hohe soziale Ressourcen. Es zeigte sich kein signifikanter Einfluss der Besuchsregelungen auf das Stillverhalten, die Bindungsqualität, die Traumatisierung oder Hinweise auf eine postpartale Depression. Auch hinsichtlich der Mutter-/Vater-Kind-Beziehung ergaben sich keine signifikanten Unterschiede. Insgesamt waren die Eltern während der Covid-19-Pandemie stärker belastet als Eltern vor der Pandemie. Diese Belastung ließ sich jedoch nicht auf die Besuchsbeschränkungen, sondern auf die Gesamtsituation der Pandemie zurückführen.

| Translated abstract: | Abstract Language When the first lockdown was imposed in Germany on March 16, 2020, due to the COVID-19 pandemic, visiting regulations in maternity wards and pediatric hospitals were also restricted. The aim of this study was to investigate the potential effects of these access restrictions on parents of premature infants and sick newborns with regard to breastfeeding behavior, psychological well-being, parental stress, and parent-child bonding.
This was a monocentric, descriptive, prospective study conducted between August 13, 2021, and July 13, 2023. Two cohorts were compared: from March 16 to May 18, 2020, parents were only allowed to visit their child separately at the University Hospital Cologne; from May 19, 2020, both parents were again permitted to visit together. After providing informed consent, parents received validated questionnaires, including the F-SozU, EBI, IES-R, EPDS, and PBQ, as well as questionnaires developed by the neonatal team regarding socioeconomic characteristics and breastfeeding behavior. Clinical data of the infants were obtained from the hospital's internal databases. Statistical analysis was performed using SPSS version 29.
A total of 21 parents from the period with strict access restrictions and 15 from the period with relaxed restrictions participated in the study. The two study groups were comparable with regard to infant and parental baseline characteristics as well as socioeconomic status. Both groups reported high levels of available social support. No significant effect of the visiting regulations was found on breastfeeding behavior, parent-child bonding, traumatic stress, or symptoms of postpartum depression. Likewise, no significant differences were found in the parent-child relationship. Overall, parents during the COVID-19 pandemic experienced greater psychological burden than parents before the pandemic. However, this increased burden could not be attributed to the visiting restrictions themselves, but rather to the overall circumstances of the COVID-19 pandemic. English |
