Secondary prevention after rehabilitation is insufficient in orthopedics as well as cardiology. Through the continuous spread of mobile, internet and multimedia services, it is increasingly apparent that it is possible to effectively use new electronic media possibilities for a person-centered and effective follow-up care. Internet-based applications like email, asynchronous e-learning or chat are increasingly gaining significance. This article presents the results of the project Tele-Assist. In the project Tele-Assist, based on national and international guidelines in orthopedics and cardiology, and on the basis of the MeineReha (R) system, a digital and interactive therapy program for rehabilitation was developed and tested for feasibility in two clinics. Subsequently, the evaluation results with regard to acceptance, motivation and patient safety are listed.
