Rescue of patients from difficult locations is technically complex. Safety of work for the recue team is of utmost importance. This case report presents a 52-year-old construction worker with cardiac arrest at a height of 14 m on a roof. Despite initial return of spontaneous circulation, the patient presented with hemodynamic instability and re-arrest. After extensive prehospital therapy, the decision was made to transport the patient using a mechanical cardiopulmonary resuscitation (CPR) device (LUCAS (TM) 2). In the case report, using mechanical devices for prolonged CPR is also critically discussed.
