Abstract

Background Treatment of advanced, relapse, and chemotherapy-resistant germ cell tumors is a particular challenge. Multidisciplinary treatment of patients with chemotherapy and surgical resection of residual tumor tissue results in excellent survival rates. In the rare cases of recurrent germ cell tumors, rare recurrence locations or resistance to chemotherapy, surgical resection is of particular importance. Objectives This review article describes the importance of surgical resection of residual tumor tissue in advanced germ cell tumors after first-line therapy and in the salvage situation but also in the special case of desperation surgery in the event of increasing tumor markers during or after salvage chemotherapy. Materials and Methods A selective literature search in PubMed was conducted Results Surgical resection of residual tumor tissue >1 cm after chemotherapy is an integral part of guideline-based therapy for nonseminomatous germ cell tumors and lowers the recurrence rate. In advanced seminoma, residual tumor resection is usually not indicated. After completion of salvage chemotherapy, residual tumor resection must be performed regardless of the size of the residual tumor. Conclusion With a long-term survival of about 50%, desperation surgery for patients with rising markers after or during salvage chemotherapy represents a valid surgical option for chemotherapy-resistant testicular cancer. However, histologically complete resection is prerequisite for oncological success.
