Abstract

Background Lateral epicondylitis, also often called tennis elbow, is a common orthopedic condition. However, nowadays a more differentiated approach is needed to understand the pathologies of lateral elbow pain. Besides the degenerative changes of the extensor, mass cartilage lesions of the radial compartment, plica hypertrophy or posterolateral rotatory instability (PLRI) may lead to lateral elbow pain and be clinically masked as epicondylitis. Diagnosis Patients present with a painful lateral elbow. In case of concomitant elbow pathologies, they should be carefully examined and the clinical test for PLRI should be considered. Generally, the diagnosis is based on the clinical examination. T2-weighted magnetic resonance imaging can be useful for chronic course (>6 months). Treatment Due to the high rate of spontaneous remission within the first 12 months, nonsurgical management including a multimodal therapy concept is the mainstay of treatment. Surgical treatment may be indicated in patients with persistent symptoms after conservative treatment has been exhausted or with concomitant pathologies. In case of surgical treatment, arthroscopy is particularly helpful to identify and address concomitant elbow pathologies.
