Abstract

The increasing knowledge about risk factors and especially high-risk genes has brought risk-adapted cancer prevention into the focus as a potentially more effective prevention concept than conventional prevention strategies that are geared to the population as a whole; however, carrying out prospective randomized studies to provide evidence for risk-adapted prevention measures is problematic, since the evidence of a significantly increased risk is associated with a low acceptance by those affected with respect to randomization. This is aggravated by the fact that genetically defined subgroups are often too small to perform randomized controlled trials (RCT) usually within a limited time frame. In this situation framework concepts have been developed that enable a structured and systematic generation of evidence. The German Consortium of Familial Breast and Ovarian Cancer has taken up these concepts and integrated them into a care concept that enables knowledge generation and at the same time nationwide networked care.
