Abstract

Background Gastric carcinoma is often diagnosed at an advanced stage so that it cannot be removed locally via endoscopic resection. Surgical resection is then the only option for curative treatment for potentially resectable gastric carcinoma. Objective Based on the S3 guidelines and the current literature, important aspects of gastric surgery for adenocarcinoma of the stomach and the gastroesophageal junction (AEG II/III) are discussed. Material and methods A literature search was carried out with respect to current aspects of oncological gastric surgery, including the current German S3 guidelines Results and conclusion Minimally invasive surgical procedures play an increasingly important role in oncological gastric resection. For early gastric carcinoma of the distal third of the stomach, studies in particular from East Asia have shown that laparoscopic resection is equivalent to open resection under oncological aspects. Laparoscopic resection of advanced carcinoma is currently still in the evaluation; however, it is also increasingly evident here that laparoscopic resection of distal carcinomas is equivalent to open resection with respect to oncological endpoints. Robot-assisted surgery has not yet become established as a standard in oncological gastric surgery, particularly due to the costs and the longer average operating time.
