Abstract

Background The exact histomorphological and molecular characterization of the different subtypes of gastric cancer is becoming increasingly important with regard to personalized therapy options. Some subtypes are frequently associated with potentially therapy-relevant biomarkers. The pathologist also plays a decisive role in the determination of these biomarkers. Aim The 2019 updated WHO-based classification of gastric carcinomas, molecularly defined subtypes and potentially therapy-relevant biomarkers that the pathologist determines in gastric carcinoma are reviewed. Materials and methods A selective literature search, especially with regard to current tumor classifications and biomarker determinations, was performed. Various databases such as PubMed and WHO were used. Conclusions Some morphologically defined tumor entities (e.g. gastric carcinoma with lymphoid stroma) can be assigned to molecularly defined gastric carcinoma subtypes (e.g. Epstein-Barr virus-associated gastric carcinoma). The morphological consideration of the inflammatory tumor micromilieu (e.g. lymphocytic inflamed tumors, PD-L1 expression of certain inflammatory cells or carcinoma cells) can be an indication of an underlying microsatellite instable (MSI) gastric carcinoma. Both entities show an increased response probability to immune checkpoint inhibitors. Pathology plays the decisive role in the determination of these subtypes and opens up personalized therapy options. All metastasized gastric carcinomas should be exactly characterized, including molecular characterization. Thereby, therapy-relevant biomarkers should also be determined. This can be done cost-effectively and comprehensively with standard pathology methods.
