Abstract

The combination of D-dimer test and clinical signs of the Wells rule is sufficient to exclude deep-vein thrombosis in non-oncological patients, as again proved by a large meta-analysis. In these cases, time-consuming duplex ultrasonography is unnecessary. After being controversially discussed 15-20 years ago, outpatient treatment meanwhile is favoured in approximately 95% of the cases. Since development of ultrasound scanners is improving steadily, venography is losing its significance as a reference-tool in the diagnosis of deep-vein thrombosis. In recent years the approval of three oral anticoagulants rivaroxaban, apixaban and dabigatran (after day 5) in addition to the established agents fondaparinux, phenprocoumon, warfarin, low-molecular-weight heparin and unfractionated heparin has changed outpatient treatment of deep-vein thrombosis. Several new anticoagulants will follow, their significance in daily routine and the value of their different dose schemes during the treatment-period will have to be defined yet. Individual aspects are becoming more and more important in deciding on the treatment option. Effective compression therapy of the lower limb remains essential after deep-vein thrombosis, even though a recent clinical study could not illustrate its effect.
