Abstract

Background Since the end of the 1970s, drug data from health insurance funds have been used to analyze the development of consumption quantities and costs. The aim of the article is to show the spectrum of questions of studies in drug consumption research and pharmacoepidemiology that were processed on the basis of health insurance fund data. Materials and methods The starting point is internationally referenced publications (MEDLINE) since 1975 with data from Germany, which were evaluated with regard to the questions addressed, the chosen study design, and methodology. Results Data profiles and variables used in 235 drug-related studies are outlined and methodological requirements for the use of drug data are shown. For descriptive studies, these refer to aspects of the data source, the definition of the study population and to questions that have to be clarified in advance when using drug data. In the case of analytical studies, particularities with regard to the determination of exposure as well as indication and outcomes are addressed and relevant literature on research design and control of bias and confounders is pointed out. Discussion and conclusion Studies on drug consumption research have so far mainly been conducted on the data sets of individual health insurance funds. The Research Data Center Health of the Federal Institute for Drugs and Medical Devices (BfArM), which is currently being established with data of all persons insured by statutory health insurance (SHI), could bring about an improvement of the data situation, as this avoids health insurance fund-specific peculiarities. Rare events that require large data sets could also be investigated using this data set. Links with other data sources should be provided.
