Abstract

Objective: This work aimed to investigate and to describe which interventions including physical activity for PD exist via a systematic review. The network meta-analysis assessed which physical intervention is the most effective to improve QoL in patients with PD. Design: Systematic review and network meta-analysis of randomized controlled trials. Methods: Eligible literature were retrieved from PubMed up to February 2018 and screened based on established selection criteria. Afterwards, relevant data and study characteristics were extracted. The network meta-analysis was conducted with the software R-Studio to evaluate the effect of different sport interventions. Risk of bias was performed for the included studies. Results: 52 trials of 3653 participants met the inclusion criteria for the systematic review, of which 21 trials provided data for the meta-analyses. 21 studies (50%) assessed QoL using the PDQ-39. 26 trials in the systematic review reported a significant benefit from physiotherapy. In the network meta-analysis all interventions except for trunk exercises (MD: 4.67; 95%-CI [8.98; 18.32], bicycle (MD: 1.35; 95%-CI [-5.67; 8.38], motor education (MD: 3.34; 95%-CI [16.05] and broader senior sports (MD: 5.38; 95%-CI [-12.04;22.80]) exhibited an increased efficacy compared to usual care due to PDQ-39. Hydrotherapy was the most helpful (MD: -13.49; 95%-CI [-27.54; 0.56]) compare to usual care as reference. Conclusion: There is a wide variety of interventions using physical activity for PD. Hydrotherapy, exergaming and Nordic Walking had the biggest effect on QoL in PD. To make reliable statements of the efficacy and cost effectiveness of physiotherapy for treating PD in the longer term larger, well-designed RCTs are required. This work provides the basis for further research.
